MAKE AN APPOINTMENT ?

Patients routinely book 10 minute appointments. If you think you will need longer please ask for extra time.

Sick Children: We have a "No Waiting" policy for babies and young children.

Please ask for an "urgent" appointment for that day. You will be seen the same day, but not always by the doctor of your choice.

OBTAIN A HOME VISIT ?

Please state that the visit is needed "urgently". The request will then be dealt with immediately.

OBTAIN A REPEAT PRESCRIPTION ?

Attached to your prescription is a request form for re-ordering, mark the items required. To order a repeat prescription either please post it to the surgery, leave it in the repeat prescription box by the dispensary, fax it to us on 01935 817484 or email your request to:- Apples.Prescriptions@gp-J81029.nhs.uk or simply in writing. The prescription can either be picked up from the surgery 24 hours later OR from your chosen specified chemist a further 24 hours later. If you wish the prescription to be posted back to you please enclose a SAE.

Patients who live more than one mile from a chemist in a rural area can have the medication dispensed directly from the dispensary in the surgery.

We have now installed a dedicated telephone line for prescription requests. This telephone line is manned from 09.30-11.30 Monday, Tuesday and Thursday. The telephone number is 01935 816595.
