The Apples Medical Centre - Introduction for New Patients
Welcome to The Apples Medical Centre which is a practice with around 5,400 patients currently registered.
This Introduction for New Patients has been produced by the PPG (Patient Participation Group) as a guide for new patients to The Apples Medical Centre.
The Apples Medical Centre is in East Mill Lane, Sherborne DT9 3DG. There is pedestrian access from Long Street.
As a new patient, you can either register at Reception and complete a paper form, or you can register online via the surgery’s website (https://www.sherborneapples.co.uk). If you are on existing medication, a clinician will contact you.
The surgery has 4 GPS, an Advanced Nurse Practitioner, a Managing Partner, Practice Nurses, HCAs, Phlebotomists, a Medical Administrator/Secretary, Dispensers and Medical Receptionists. The surgery is part of the Sherborne Primary Care Network (The Grove Medical Centre and Yetminster Health Centre) - within the PCN, there are the following roles: Mental Health Nurses, Social Prescribers, a Frailty Nurse, and Pharmacy Team.
District Nurses and a Health Visiting Team are located at the Yeatman Hospital. There is also a Midwife (who attends the surgery fortnightly on Tuesday mornings), a Physiotherapist and Cancer Care Nurse, as well as a Community Mental Health Team and Clinical Pharmacists.
Appointments
To request an urgent appointment on the day, please telephone Reception on 01935 812633 or visit Reception between the hours of 8 a.m. and 6.30 p.m. Monday to Friday. To request an appointment in the next 14 days or a routine appointment within 4 weeks, use an eConsult form, ring 01935 812633 or visit the surgery.
When you contact the surgery, you will be asked what you need help with. This information will be used to choose the most suitable health professional to direct you to.
To cancel or change an appointment, use SystmOnline or contact the surgery between 8 a.m. to 6.30 p.m. Monday to Friday.
If you need help when the surgery is closed, use NHS111 or call 111, or if it is life threatening, call 999.
Repeat Prescriptions
You can request repeat prescriptions online via the NHS app and using the GP online system, SystmOnline. Alternatively, you can visit the surgery from 8.30 a.m. to 6.30 p.m. Monday to Friday or telephone 01935 812633 and press option 2 for dispensary which is open on Mondays, Tuesdays, and Thursdays from 9.30 a.m. to 11.30 a.m. You can also post your repeat prescription or email apples.prescriptions@dorsetgp.nhs.uk using the title Repeat Prescription Request. Your prescription will be ready for collection FOUR days later either at the surgery or from your chosen specified chemist.
Test Results
During your blood test appointment, you will be asked if you have access to the NHS app/SystmOnline. If you do, you will be directed to view 5 to 7 days from the date of your test. If you do not have access, you will be informed that if your test is abnormal, you will receive either a SMS/email or telephone call. For all normal tests, patients will not be informed.

To register for online services, contact the surgery for details or register online. Once registered, you can use SystmOnline.
