Patient Registration

To register online with the practice please click on the link below and select the Register with the Apples Medical Centre option.

Complete the online GMS1 form in as much detail as possible and click the submit button when you have finished; then print the form and sign it, or you can save the document as a PDF and electronically sign it and email it to the surgery. DO NOT post your GMS form you must bring it with you to the surgery along with your ID and proof of address.

After completing your GMS form and New Patient Questionnire you MUST present to the surgery with a valid form of photo identification (e.g. Driving License or Passport) and proof of address (e.g. utility bill, council tax statement etc.) You MUST also bring your printed or hand written signed GMS form. If you do not bring your GMS form, you will be required to complete another one at the surgery.

Once we have fully registered you a new patient health check appointment will be made for you with the practice nurse, or doctor if you need your medication reviewing.

At the time of registering we will also ask if you would like to register for the SystmOnline service. This online self-service facility enables you to order repeat medications and book appointments from the comfort of your own home using laptop or desktop computer, or when you're on the move via a tablet or smart phone.

If you do not present to the surgery with valid photo ID and proof of address within two weeks of registering online, your registration will be cancelled.
